Never Let a Good Crisis Go to Waste: Leveraging External Failures to Drive Measurable Safety Improvements in Radiology
In radiology, even one missed opportunity to learn from failure can have serious consequences. At Southwest Medical Imaging (SMIL), we’ve taken the concept of "never let a good crisis go to waste" and built a proactive framework around it through the creation of our Failure Analysis Critique Team (FACT). FACT is a multidisciplinary committee that conducts root cause analyses (RCAs) on critical incidents not just those occurring within our walls, but high-impact events reported across the county, state, nation, and world. By asking “What if this happened here?” we uncover latent vulnerabilities in our own systems and implement real-time solutions before harm occurs.
